Online 'Pre-Registration' With The Practice

Please note Patients are required to bring proof of photographic ID and proof of address to the practice and sign your registration form before you are registered.  

If you wish to pre-register click on the link`s below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct. The medical questionnaire also needs to be filled in along with the Patient policy on drugs liable to misuse for any patient to be registered at Royston Group Practice. This is because it can take a considerable time for us to receive your medical records.
